The day before surgery

The day of your surgery
After you have arrived, you will be given something to relax. As the time for your surgery approaches, your surgeon will be with you as you are put to sleep under general anesthesia. SRS MTF surgery will begin. Once your surgery is completed, you will wake up in the recovery room. You’ll feel a feeling of fullness from the catheter and vaginal packing.
After your surgery
We’ll start by keeping your pain under control while resting, recovering, and healing. You will be allowed to sit up in bed immediately, get out of bed, and start to walk on the second day. You’ll have your dressings changed, and you’ll continue to walk a little more each day. On the seventh day after surgery, you have the vaginal packing and bladder catheter removed, and we’ll go over your care routines of dilation and douching. Finally, on the ninth day after surgery, you’ll be released to go home with your Surgical Declaration letter.
Once you return home from the hospital, you may see some bleeding, oozing, or discharge from the incision sites or blood signs when a catheter is removed. If you experience bleeding, light pressure using a fingertip over the site for five minutes should stop it. Expect the wounds to look worse before they look better, with improvements beginning approximately 2-3 weeks post-surgery. The swelling can take months to subside.
It is important to stay alert to signs of complications. Though rare, if you notice any of the following signs, call the surgeon’s office immediately:
- Significant bleeding not stopped with pressure
- Foul-smelling or pus-filled discharge
- Fever over 101° F
- New or significant pain
- Wound separation
Dilating and Douching

Expect some slowness due to pain medication. Do not strain. Use an OTC stool softener like Miralax.
Resuming Activity After GRS
- Sex – For those who have undergone GRS MTF surgery, you may have penetrative sex using a lubricant beginning 12 weeks post-surgery.
- Exercise – You can start gentle exercise after four weeks but restrict all other exercise or outdoor activities until your surgeon releases you.
- Swimming – Hot tubs, swimming pools, or open water is out of the question for the first 12 weeks.
