When we gain or lose weight, we do not add or subtract fat cells, as is commonly believed. Rather, we change the amount of fat within each cell. While fat cells lie beneath the skin throughout the body, they accumulate in greater amounts in certain areas. This explains why many individuals who experience weight loss are still left with bulges or fat deposits in certain areas of the body.

Liposuction is a surgical technique in which special probes, attached to a high suction pump, are passed through tiny punctures made in inconspicuous places around the body. As fat cells are suctioned out, the surgeon shapes the area, ensuring an aesthetically pleasing appearance. In some cases, liposuction may be followed by surgery to remove excess skin. These specific procedures are outlined below.

Liposuction can be performed on virtually any area of the body, and is particularly useful in the neck, abdomen, waist, hips and thighs. In addition, it can be used on the arms, buttocks, calves, and ankles. If very large volumes of fat are to be removed, it may be necessary to carry out more than one operative procedure over time.

During the procedure, a small incision is made in the area of fat to be removed. A long thin needle is used to inject a liquid called "tumescent,” which makes it easier to remove the fat with less bleeding, postoperative bruising and pain. Following the introduction of tumescent, a thin, blunt instrument is inserted under the skin, through the same incision, and carefully manipulated through the fat tissue. The probe is attached to a high vacuum hose, which suctions out the unwanted fat cells. After the desired amount of fat is removed, the incisions are closed and dressings applied.

Liposuction is most often performed under general anesthesia. Following the procedure, patients are instructed to wear a pressure garment to reduce swelling and aid in the healing process. Side effects, which are temporary and expected to last for several days post-surgery, include swelling, bruising, discomfort and numbness.

Adominoplasty (Tummy Tuck)
Adominoplasty is a procedure whereby excess fat and skin are removed from the abdomen and, simultaneously, the abdominal muscles are tightened. The result is a smoother and firmer appearance.

Tummy tucks are popular procedures in individuals who have experienced pregnancy or significant weight loss. During the procedure, incisions are made along the lower abdomen and around the belly button. The skin and fat are elevated and repositioned as necessary and the abdominal wall is tightened through the use of stitches. Excess skin and fat is then removed and remaining skin is stitched into position. The belly button is pulled through the new skin via a small incision. It is then stitched to the new skin, to ensure proper positioning.

Abdominoplasty is generally performed under general anesthesia. Occasionally, patients must spend a night or two in a post-surgical facility. Post-surgery, patients’ physical activities are limited. Within four to six weeks they may return to normal activities. Side effects may include significant swelling and temporary pain and numbness.
